Three Rivers Community Health is a federally qualified health center with two locations in Lyles and Linden, Tennessee.  All insurance is accepted including patients without health insurance which are charged on a sliding-scale fee based on the patient's household income.  Uninsured patients are asked to bring with them verification of income such as pay stubs, W-2, SSI benefits statement, Food Stamp letter or card, etc.
