By popular demand, we will now take back any DOH medications that have expired on your shelves.  From this point forward, all orders will include an Expired Drug Return Form on which you will list each drug and quantity being returned, the serial numbers found on the yellow zip ties used to secure the bin, your site name, contact number, and signature.  If you need additional bins, please call (888)428-HOPE(4673) and provide Kathy your dispensing site name and the number of bins needed.  BE SURE TO INDICATE THE BINS ARE FOR YOUR EXPIRED DRUG RETURNS.
